General Liability Release

I understand that my participation in the Possibilities® seminar is wholly voluntary.

This seminar is not a form of treatment or therapy for any mental, emotional, or physical problems of the undersigned. This seminar is for educational purposes only.

I agree to observe and obey all posted rules and warnings, and further agree to follow any oral instructions or directions given by Possibilities Instructors, employees, representatives, or agents during any voluntary participation in outdoor activities including but not limited to white water rafting, kayaking, hiking, etc...

I recognize that there are certain inherent risks associated with the above described activities and I assume full responsibility for personal injury to myself and (if applicable) my family members, and further release and discharge Possibilities Consulting, LLC for Injury, loss or damage arising out of me or my families use of or presence upon the facilities of possibilities consulting LLC, whether caused by the fault of myself, my family, Possibilities Consulting LLC, or third parties.

In consideration of my being permitted to participate in this seminar, I hereby release, hold harmless and indemnify Possibilities Consulting LLC, its successors and assigns, staff, personnel, and assist team members from all claims made by me, or on behalf of myself or my estate, for any illness or damage arising from my participation in such seminar or related services and activities. I assume full responsibility for any and all effects of my taking part in this program.

Mutual Confidentiality Agreement

I understand and agree that all records, files and notes relating to my participation in this seminar shall be forever confidential and shall be made available only to Possibilities LLC personnel to be used exclusively for my benefit as a participant in such seminars and associated activities.

I further understand that participation in this seminar is a private experience for each participant. As a consequence, I agree to hold confidential all remarks and behavior of all participants; to treat as personal and private all their comments, experiences, and interactions in the seminar process; and to refrain from discussing with anyone outside the training environment anything that another participant said or did while enrolled in the program.

Release of Liability

Electronic Signature Agreement. By selecting the "DIGITALLY SIGN / SUBMIT" button, you are signing this Agreement electronically. You agree your electronic signature is the legal equivalent of your manual signature on this Agreement. By selecting "DIGITALLY SIGN / SUBMIT" you consent to be legally bound by this Agreement's terms and conditions. You further agree that your use of a key pad, mouse or other device to select an item, button, icon or similar act/action, or to otherwise provide Possibilities Consulting, LLC, or in accessing or making any transaction regarding any agreement, acknowledgement, consent terms, disclosures or conditions constitutes your signature (hereafter referred to as "E-Signature"), acceptance and agreement as if actually signed by you in writing. You also agree that no certification authority or other third party verification is necessary to validate your E-Signature and that the lack of such certification or third party verification will not in any way affect the enforceability of your E-Signature or any resulting contract between you and Possibilities Consulting, LLC. You represent that you are authorized to enter into this Agreement.
